Instructions
1. You have to pay tuition fees as printed on fee challan at any designated branches of Axis bank or by online
payment for Medical & Dental courses between 06/11/2017 to 07/10/2017 up to 4:00 PM only (Except Sunday
& Holiday) in normal banking hours. If you fail to pay the required first term tuition fees within prescribed time
limit then this allotment will be cancelled and you will not be eligible to get this seat in the subsequent rounds
of counseling, if any. In case of refund of fees, if any, it will be refunded after completion of admission process
by Demand draft/Cheque in the name of student. ACPUGMEC will not be liable for any discrepancy related to
fees paid in other than designated branch of Axis bank.

2. After payment of fees either by paying on designated branch of Axis bank or by online payment gateway
within prescribed time limit, you have to visit nearby Help centre to confirm your allotted admission with
following original documents otherwise your admission will not be confirmed and get cancelled.

3. For SC students having family annual income less than Rs.2,50,000/- must submit Freeship card for Zero
fee benefit.

F. Document showing place of birth & date of birth & Indian Citizenship (School leaving Certificate/ Transfer
Certificate / Passport / Birth Certificate) G. Copy of Passport [if Citizenship is Dual / Foreign]

H. SEBC Cast certificate issued by Authorities of Gujarat State only

I. Non-creamy layer certificate (Parishistha ‘4’ in Gujarati/English) issued by Authorities of Gujarat State only
