AFFIDAVIT OF DISCREPANCY

I, JAYSON P. TOMAS, Filipino, of legal age, married, Filipino and a

resident of A. Bonifacio St., Libis, Benguet, Rizal, after having duly sworn to in
accordance with law hereby depose and say:

1. I am the accused in Criminal Case No. 12345 entitiled “People of the

Philippines versus JAYSON Ttomas y PABRO for the offense of “Child
Abuse”, pending before the Regional Trial Court, Branch 75, Benguet,
Rizal, which was provisionally dismissed on November 10, 2014;
2. I posted cash bond for my provisional liberty on April 24, 2013 in the
amount of Php 30, 000.00 under the name of JASON P. TOMAS;
3. The names JAYSON TOMAS Y PABRO and JASON P. TOMAS refer
to me, being one and the same person;
4. I am executing this Affidavit of Discrepancy to attest to the truth of the
foregoing facts and in support of my application for the correction of all
my private and public records.
