Application Checklist – Please submit the following items with your residential loan application:

☐ Pay Stubs: Current pay stubs for the most recent 30-day period. If you receive bonus or commission
income, provide prior two year’s end of year paystubs showing annual amount received. If retired,
provide annual retirement statement and social security statement if applicable. Please include an
explanation for any gaps in employment of 30 days or longer.

☐ W-2/1099 Forms: IRS W-2 and 1099 forms for the most recent 2 years.

☐ Federal Tax Returns: Personal and Business federal tax returns for the most current two years – all
pages, statements, schedules and K1’s if applicable. Please sign and date prior to submitting.
*** If you are a first time homebuyer, please provide three years Federal Tax Returns ***

☐ P&L Statement, Balance Sheet and GE Tax Filings: If you own more than 25% of a business,
provide a year-to-date profit and loss statement and balance sheet. Also, current tax year GE Tax Filings.
Please sign and date prior to submitting.

☐ Bank Statements: Current bank statements (all pages) for the most recent two-month period or
quarter, including investment and retirement accounts – with withdrawal terms if investment/retirement
funds will be used for closing. If business bank statements are provided, six months are required.
*** Source and explain all large deposits that are larger than 50% of your gross monthly income ***

☐ HOA Monthly Dues Statement: For all owned properties with monthly association dues.

☐ Landlord Information: If you are currently renting a home, please provide the name, address and
telephone number of your landlord(s) for last 24 months. Also provide your lease agreement if available.

☐ Trust Documents: For properties to be held in Trust- Long Form is required.

☐ Rental Agreements: If you own rental properties, provide current rental agreements on all rental
properties.

☐ Homeowner’s Insurance: Insurance declarations page for all owned properties.

☐ Mortgage Statements: Current mortgage statements for all owned properties. If refinancing, please
consider cancelling all Autopay features on your current mortgage(s) to be paid off and make any
required payments manually by the due dates.

☐ Creditor Statements: Current statements for any credit accounts to be paid off at closing of your
refinance loan. *** Please do not apply for any new credit until after your loan closes ***

☐ Closing Statement: If you purchased or sold a home within the last 24 months, provide your closing
statement for the property purchased or sold.

☐ Sales Contract: If purchasing, provide a copy of your executed sales contract and all addendums.

☐ PV System: Provide evidence and documentation to verify if the system is owned or leased.

☐ **Valid ID’s for internal verification purposes
