Subject: Application for seeking information under RTI Act, 2005

Dear Sir,

Under the provisions of RTI act 2005, please provide the following information:

1. Please provide an attested copy or original file/documents submitted to the electricity

office for CS electricity connection at my shop. Please provide me only original
submitted documents or an attested copy of it. My electricity connection details are
following.

JE AREA: LALHARI.
2. As now I have came to know from electricity office that above mentioned electricity
connection has been permanently disconnected with the reason PAYMENT
DEFAULTER. But why I was not informed and why the disconnection notice of
permanent disconnection has not been served to me.
3. Why 15 days of disconnection notice has not been served to me for not paying bill’s.
4. If any permanent disconnection notice has been sent to me, please tell me the date of
sending the notice or receiving the notice by me and By which method the permanent
disconnection notice was served to me (by POST or verbally on phone or verbally by the
officials).
5. Please tell me the name of the person who received the notice, as neither I nor any
member of my family was given notice, either verbal or in written.
6. If the Electricity Board officials had informed me by phone then please provide me the
phone number on which they had called me.
7. If the Electricity Board officials had given the permanent disconnection information
verbally, then provide the name of the person to whom they had given the information.

8. How can any other person sign the Permanent Disconnection Order form as a customer
and disconnect the electricity connection of my shop without my approval or transfer of
ownership of my shop’s property and apply for a new electricity connection at the same
place. Copy of permanent disconnection form is attached.
9. If any document bearing my name or my signature was used for permanent disconnection
of electricity connection of my shop, please provide me with an attested copy of the same.
10. Mention the date of permanent disconnection of electricity connection of my shop.
11. As per HPSEB rules given on their website, was my shop’s electricity connection
disconnected temporarily due to non-payment of bill’s or not.
12. Was default in payment(s) continued for a period of six months, from the date of payment
first became due.
13. On what basis a new electricity connection allotted to another person at the my shop?
Since I have not sold my shop property to anyone.
14. Please provide the document which was used to allot the new connection at my shop to
another person, provide me with an attested copy of the same.
15. Is there any N.O.C attached with the above said new connection if yes then please
provide an attested copy of it.
16. Mention the installation date and customer name of new electricity connection in same
shop.
