ADENOCARCINOMA- Adenocarcinoma is a type of cancer that may affect various organs.

It is
derived from the word “adeno” meaning 'pertaining to a gland' and “carcinoma” meaning
cancer.Adenocarcinoma is a type of cancer that starts in glands that line the insides of the
organs, which is why it can affect different body areas. Adenocarcinoma forms in glandular
epithelial cells, which secrete mucus, digestive juices or other fluids. In some cases,
adenocarcinoma spreads to other parts of the body, and at that point, it's called metastatic
adenocarcinoma.

B. HISTORY OF PAST ILLNESS


The patient has a hypertension and has a maintenance of amlodipine and losartan

C. HISTORY OF PRESENT ILLNESS


● October 2023- patient had episodes of epigastric pain heavy 5/10. Associated with
occurrence vomiting coffee ground ½ cup, melena semiformed sought and given
omeprazole
● November 2023- no recurrence of symptoms
● December 2023- patient had recurrence of epigastric pain, vomiting of coffee ground
melena & black stool sought consult admitted and was given 3 units PRBC,discharge
referred.
● January 2024- patient sought consultation still with above symptoms endoscopy
done,biopsy done and advised for possible surgery.
● Persistence of occurrence vomiting & black stool. Consult hence admitted
