MRI OF LUMBER SPINE

Sequence protocol: Sagittal - T1, T2, STIR

3D Myelogram sequence
Observations :

Lumber column shows normal lordosis.

Wedge fracture of L1 vertebral body with marrow edema noted. Left superior endplate irregularity
with marrow edema seen in L2 vertebral body.

Vertebral bodies otherwise show normal morphology and marrow signal characteristics.

L1-L2 & L4-L5 intervertebral discs are desiccated.

Circumferentially bulged L4-L5 disc with posterior annular fissure ,broad
based posterior central protrusion, bilateral paracentral and foraminal bulge causing ventral
thecal column indentation , mild bilateral foraminal stenosis & mild compression of L4 exiting
roos (L > R) . L1-L2 disc height is diminished.

Rest of the lumber intervertebral discs are normal.

Facet joints and ligamenta flava are normal.

Conus and cauda equina are normal in morphology & signal characteristics.

Posterior longitudinal ligament is normal.

SI joints are normal.

IMPRESSION : MRI features suggest :

* Genant grade I acute collapse of L1 vertebral body.
* Lumber degenerative disease.
* Circumferentially bulged L4-L5 disc with posterior annular fissure ,broad
based posterior central protrusion, bilateral paracentral and foraminal
bulge causing ventral thecal column indentation , mild bilateral foraminal
stenosis & mild compression of L4 exiting roots (L > R) .
- Clinical correlation suggested.
