4. Checklists for each class
5. Monthly Teacher’s checklist for each class
6. Log of sessions
Format for documentation of group sessions:
• Date
• Number of children present
• Goal
• Objective
• Methods/Techniques used (Creative expressions used by the therapist should be
mentioned)
• Session details
• Outcomes (responses to be recorded in tabular columns employing the Likert scale)
The responses in relation to the objective and the core music therapy activity have to be
recorded mandatorily.
The key observations and any progress noted for each child should be mentioned
descriptively.

Columns to be added:

• Methods/ Techniques used
• Progress observed
• Comments (to be filled by the internship supervisor)
