Clinical background:
 ODS.

MRI FINDINGS:
Static MRI Findings:
 Resting dilated rectum (mega rectum) with retained fecal matter
and markedly destined sigmoid colon.
 Intact anal sphincteric complex. No defect or scarring.
 Preserved muscle bulk of the puborectalis and iliococcygeus
muscle.
 No endoplevic fascia defect or scarring.
 Unremarkable MRI appearance of the urinary bladder and uterus
with no gross lesions.
MRI Defecography Findings:
 No abnormal urinary bladder base or uterine descent below the PCL.

MR

 Descent of the ARJ below the PCL about 2 cm.


 Focal anterior rectal wall bulge measuring about 3.2 cm.
 Evacuation was slow and incomplete. No evidence of rectal
intssusception or internal rectal prolapse.
 Persistence pubrectalis muscle indentation on the posterior aspect
of the ARJ with inadequate descent of the pelvic floor associated
with inadequate opening of the anal canal.
 No evidence of enterocele,sigmoidocele or peritoneocele.

Dynamic MRI Findings:


 Normal Levator hiatus width about 5 cm.
 Mild abnormal elongation of the iliococcygeus slings,the
iliococcygeal angle measuring about 55o.
 No abnormal levator plate sagging with levator plate angle of 17o
(normal 11.7+/-4.8 °).

Opinion:
MRI features suggestive of;
 Dyskinetic puborectalis (spastic
pelvic floor syndrome), for further
clinical and manometric
correlation.
 Markedly dilated rectum and
visualized part of the sigmoid colon
