Hi Chun, the member signature date seems invalid (11th April 1983) on beneficiary designation
form. However, Declaration of marital status form signed with date 11th April 2024. Please
review and advise can we update the beneficiary in Ariel as it seems member mistakenly
mentioned spouse date of birth year (1983 instead of 2024). Thank you!
