OVERALL REVIEW:
 Delimit:
Beaconing/restricting the work area in lifting maneuvers, to prevent falling objects on workers at lower levels, avoid collisions between machinery, etc.
 Order and cleanliness:
The work area should be kept neat and free of elements to prevent accidents, stepping on objects, and maintaining all free passage for emergency evacuation.
 Manual ladders (according to activity):
The ladder should be firmly resting on the floor, with non-slip studs. An operator holding it from the ground, if possible anchored at the top to avoid movement.
Additional protection shall be used for heights greater than 2 m (foot distance to ground).

ADDITIONAL OBSERVATIONS/RECOMMENDATIONS:
1. Scaffold tag was blank.
2. Excess scaffolding materials obstructed the workers’ movement.
3. Cement water made the access slippery and muddy.
