JSA by: - Coveralls (Nomex Delta C).
Job Supervisor: - Safety Helmet with Chain Guard
(No older then 3 years from date of issue).
EXTRA ORDINARY APPROVALS (ONLY IN VERY SPECIAL CASES REFERRED BY A JOB SUPERVISOR):
- Safety Shoes (Sole must not be smooth).
Discipline Superintendent - Safety Glasses (Dark & Clear with carry
Safety Representative Case).
- Hearing Protection.
Any Other Approving Authority
- Barricade/ Sign the area.
- Pre-Start Safety Toolbox Talk.
- Rigging Jacket
- PTW
- LOTO

Emergency Situation /Evacuation Gas leak /Fire In case of Emergency situation all work must be stopped,
equipment to be switched off and all personnel’s must
report to the nearest muster point considering wind
direction.

Note: 1) If any of the tasks involves biological, physical or chemical hazards, then Industrial Hygienist must also sign this Form, against “Any Other Approving Authority” above.

1. Shifting of Material / Transportation. 1.1 Personnel injury. 1.1.1 Secure fitting properly.
1.2 Manual handling injury. 1.2.1 Avoid moving the heavy load above any existing
equipment if possible.
1.3 Slip/ Trips / Fall.
1.2.2 Act as per approved rigging plan.
1.4 Property damage.
1.2.3 Good House Keeping (a place for everything and
everything in its place always).
1.3.1 Ongoing at all times as you work, the area shall
be cleaning and unwanted materials to be removed.
1.4.1 Check the area before placing the material on
the ground.

2. Lifting / Positioning of Crane. 2.1 Property damage. 2.1.1 Position the crane carefully. Check the area and
position the crane with perfect level.
2.2 Slips / Trips / Fall.
2.1.2 Position the abundant material in proper position
2.3 Falling Objects. and use wooden battens to keep them in level.
2.2.1 Barricade the area and avoid moving under the
load.
2.3.1 Only qualified Riggers to carry out work.

3. Loading / unloading Vehicles. 3.1 Personnel Injury. 3.1.1 Only qualified Riggers to carry out work.
3.2 Equipment Failure. 3.2.1 Exact capacity crane and trailer to be used.
3.3 Flying materials. 3.3.1 Use only certified and colour coded lifting gear.

3.5 Manual Handling injury.
3.5.1 Maintain manual handling techniques.

emphasize the ill effects of Heat Stress & the
mitigating controls / Prevention to be undertaken. To
rearrange working hours to avoid high temp timing,
increase resting hour’s frequency.

6.1.3 Personnel work rotation/management must be


implemented to escape from Heat Stress.
