Incident: An incident is an undesired event
that has caused or could have caused
damaged, death, injury or ill health.

 Incident types:

Near Miss: A near miss can be describe as an


incident that results in no injury or damage, but
which has the potential to do so.

Accident: An accident is an incident which results


in injury to someone or damage to property.

There are a number of reasons to investigate
incidents….

 To collect the information you need to pass on to the


enforcing authority.
 To identify the cause of the incident to stop it happening
again.
 To get information needed for an insurance claim.
 To find out the cost of an accident
 To identify any other hazards or risks and training
requirements.
 To ensure legal compliance.

Incident
 Incident are rarely caused by a single factor- there are
usually Several contributory factors. Also, these factors
will include obvious and immediate causes and less obvious
underlying causes.

 Immediate causes: Unsafe action or lack of action or


Unsafe condition.

 Unsafe action: Operating an equipment with missing


guards, bypassing interlocks, using the wrong PPE.

 Unsafe condition: Damage tools and equipment, high noise


and low lighting levels.

 Underling causes: Factors that allow the unsafe actions


and condition to happen. The majority if these are related
to the way the organization manages safety and health and
how people perceive risk.

 Root causes: Among the many contributory factors to an


incident, there will be root causes. A root cause is a factor
that may that may cause conditions that could result in an
undesirable event. If the problem were correct, it would
prevent the undesirable event from happening. In a other
words, a root cause is an event from which all other causes
spring.
