Sub: Request to surrender my old PAN No. DAHPM2231K
Respected Sir/Madam,
I have the honor to inform you that, I Eliyas Mondal, am writing this request letter to
surrender my old PAN card bearing the number DAHPM2231K. The reason being I having issued
with 2 PAN cards. I had my old PAN bearing no. DAHPM2231K for a long time, but I had
accidentaly misplaced my old PAN card.
Later, I applied for a new pan card in absence of any refrence to the old PAN No.
However, currently my new PAN No. JFGPM6176D is linked with my aadhaar no. 343831292795.
Since, I recently found out my old pan card, although my old pan card was issued with wrong
date of birth and hadn’t been linked with my aadhaar no. Hence, I decided to continue using my new
PAN No. and close my old PAN No. to all services.
Therefore, hereby I surrender my old PAN No. DAHPM2231K and request you to
kindly consider cancelling my old pan no. as I have deregistered it from all services. I shall remain
eternally grateful for this kind gesture of yours.
