SAFETY ALERT NO. 28 RELEASE DATE: MARCH 12, 2024

BRIEF DESCRIPTION: RE-ENACTMENT:


A water truck loaded with 10,000 liters of water was reversing
along a road under construction on an inclined slope to conduct
drainage de-clogging.

While reversing, the truck lost control due to brake system failure
causing it to travel 10 meters before crashing into a temporary
shed covering a manhole with five workers nearby. The accident
resulted in four of the workers being hit by the truck. One worker
was uninjured, three workers sustained injuries, and one worker
got run over which resulted to fatality.

WHAT WENT WRONG?


• Poor preventive maintenance program of the equipment
that resulted to brake system failure.
• The truck moving backward along a downhill slope leading
to a working area instead of being positioned below the
workers along the slope.
• The area where the water truck maneuvered was inclined at
a 3.92-degree angle, causing the load to shift to the back.
• Workers continued to operate on the road while the truck
was reversing towards to their working area.

RECOMMENDATION:
• Revise the work’s Method Statement and EHS Risk
Assessment to further enhance road safety program and
safe work methods.
• Conduct Re-WAT to all Involved personnel.
• Provide a standby extended hose for the water truck.
• Conduct Defensive Driving Seminar through the MDC SQTO
• Revise the Site Traffic Management and Heavy Equipment
Operation Guidelines and adopt the MEQ process regarding
inspection of vehicle.
• Share learnings company-wide through toolbox talks and
other EHS meetings.
• Conduct Flagman Training indicating the flagman’s
responsibilities of clearing access road from workers and
debris while the truck is traversing.
