1 year of which is relevant to Disaster Risk Reduction and
Management

5. Photocopy of certificate of eligibility/PRC rating and license
6. Photocopy of Transcript of Records and Diploma; and
7. Photocopy of Certificate/s of relevant trainings/seminars attended
8. Photocopy of Medical Certificate from any government medical institution within 6 months
