Risk Assessment of a Safety Issue detected based on Hazard Identification data

Name and define the Safety Issue on the SIRA sheet. Study the hazards and related scenarios. There
potential accident scenarios and each will have to be studied on its own SIRA sheet. Use as much har
estimating the input values. Observe the resulting risk values and draw the conclusions.

Safety Assessment of a (new) activity:

The study focuses on a part of the operation - usually a planned change (e.g. new route). The planned
name of the Safety Issue . Carry out Hazard Analysis, i.e. identify the hazards related to the change an
accident scenarios. Use one SIRA sheet to analyse each scenario one by one. Open new SIRA sheets
all related scenarios. The scenarios which result in inacceptable risks can be studied to identify the bes
the risk or if that is impossible, too costly or too impractical, then the decision has to be taken not to pro
change.

NOTE: In point five, the questionsanswers about the barriers follow the logic: "The barriers will fail..." --
You can easily change the wording to "The barriers will succeed..." --> "99 times out of 100".
