Each client with their accompanying adults is informed about the measures to maintain
confidentiality.
Each client should have an envelope or folder where their medical records (ITR’s),
results of laboratory examinations or other special procedures done, referrals and other
pertinent are filed. These are filed depending on a prescribed system (by numbers, family
name, barangays etc.).
Records are kept in filing cabinet with lock and key. There will be designated personnel with
access to these records. They will be only pulled out if a client provider interaction will occur
or in any situation as maybe necessary.
Personnel working outside the facility should have written request if they want to access to
the client’s records purposes of research ,follow up, etc.
A verbal/written consent of the client should be obtained before information contained in
their records will be disclosed to outside patients. The staff should not discuss the client’s
situation with non-concerned parties.
