MEMORANDUM OF UNDERSTANDING

BETWEEN

ANANTWAR EYE HOSPITAL&DONARKAR ENTERPRISES

This Memorandum of Understanding made at Nagpur onthis 17th Day of Nov 2023
Anantwar Eye Hospital Incorporated under the companies’ act 1956 and having its registration number
781 and its registered office at Nagpur herein referred to as Anantwar Eye Hospital (Which expression,
unless it be repugnant to the context or meaning there of, shall deem to mean and its successors and
assigns) of ONE PART

AND

Donarkar EnterprisesPlot no 87, Manish Layout, Behind New Hanuman Mandir, Swawalambi Nagar,
Nagpur,And having its Registration number: MH.NAG225305 hereinafter referred to as Donarkar
Enterprises(Which expression, unless it be repugnant to the context or meaning thereof, be deemed to
mean and include its successors and assigns) of the OTHER PART.

WHEREAS Anantwar Eye Hospital has agreed to enter into an MOU with Donarkar Enterprisesis desirous
to outsource Air products like : Oxygen, Nitogen etc. on the agreed terms and conditions. Now this
agreement witnessed that:

Article 1: Effective Date

1.1 The Parties hereby agree that the Effective date of the Agreement shall be the date on which the
agreement is signed.

1.2 The agreement will be valid for a period of 3 years from the effective date of agreement.
Article 2 : Services

2.1 Donarkar Enterprisesshall provide Oxygen & Nitrogen Cylenders to Anant Eye Hospital24 X 7

2.2Donarkar Enterprisesshall maintain highest standards of Quality for desired services.

2.3Donarkar Enterprisesshall abide all laws and regulations for concerned services.

2.4 All the products sent should be supervised and checked by experienced staff before dispatching.

2.5Donarkar Enterprisesshall inform delaying or non availability of the product well before to the
hospital.

Article 3: Working & Payment terms

3.1 Donarkar Enterprisesshall be responsible for transportation of the product to the hospital.

3.2 Donarkar Enterprisesshall provide service to Anantwar Eye Hospital in rate decided by hospital
&Donarkar Enterprises.

Article 4: Termination

4.1 Either party shall give the notice before 90 days for permanent discontinuation of the agreement. In
witness whereof, the parties to this MOU through their duly authorized representatives have executed
this MOU on the days and dates set out below, and certify that they have read, understood, and agreed
to the terms and conditions of this MOU as set forth herein.

The effective date of this MOU is the date of the signature last affixed to this page.
