Please submit the following requirements on or before June 3, 2027 (Monday):

For Students For Out of School Youth (OSY)
1. 16-30 years old 1. 16-30 years old

3. Barangay indigency 3. Barangay indigency
4. Current grades 4. Certificate of OSY from
barangay (1 year OSY only)
5. Proof of Income (for students/OSY) :
a. If parents/guardian are employed :
Income Tax Return (ITR) for 2023
b. If parents/guardian are unemployed :
Certificate of Non-Taxable/Wager from BIR
6. Additional requirement for private school students:
a. Affidavit of Support
b. Affidavit of Guardianship (if supported by a guardian)

Please be informed that as per the Department of Labor and Employment (DOLE) policy, the
total family poverty threshold for combined parents income is P197,868 for family with 6
members to qualify as SPES nominee.

In case you lack the necessary requirements and qualifications as abovementioned, please provide
your replacement immediately.
