SWORN AFFIDAVIT

I, MA. CRISTINA V. MIRANDILLA, of legal age, single/married, Filipino

citizen and resident of Banal St., Brgy. Poblacion, San Andres, Quezon, sworn to in
accordance with law, depose and state:

1. THAT, I am the Municipal Assessor of the Municipality of San Andres, Quezon;

2. THAT I personally know that the property registered under the name of
BIENVENIDO M. AUSTRIA embraced in Transfer Certificate of Title No.: T-21756
situated in Barrio of Pansoy, Municipality of San Andres, Province of Quezon was
transferred from Transfer Certificate of Title No. T-342598;

3. That in the instrument Bilihang Lampasan executed by Bienvenido M. Austria

signed before the Notary Public of Atty. Antonio B. Magtibay on October 27,
2010 and entered in his Notarial Register with Docket No. 315; Page No.: 25;
Book No. 734; Series of 2010 the Certificate of Title was erroneously registered
as Transfer Certificate of Title No. T-34258 instead of Transfer Certificate of Title
No.: T-21756 as evident in the Judicial Form No. 140-A CLOA Form No. 2-A
(Revised July 1999);

4. THAT, the errors committed in the instrument Bilihang Lampasan are due to
honest mistake, not intentional nor meant to commit fraud or to cause prejudice
to any third person or entity;

5. THAT, I execute this affidavit in order to attest to the foregoing narration of facts
and to explain the discrepancy in the Transfer Certificate of Title No. T-21756
appearing in the copy of Judicial Form No. 140-A CLOA Form No. 2-A (Revised
July 1999) from the one Transfer Certificate of Title No. T-34258 appearing in
the instrument Bilihang Lampasan and to declare that Transfer Certificate of
Title No. T-342598; Transfer Certificate of Title No. T-21756 appearing
in the copy of Judicial Form No. 140-A CLOA Form No. 2-A (Revised
July 1999); and Transfer Certificate of Title No. T-34258 appearing in
the instrument Bilihang Lampasan refer to one and the same property,
and for whatever legal purposes it may serve.
