Sir,
With the reference to the subject cited above, I am directed to forward herewith the
format for Self-declaration (Annexure-I) and D.D.O Certification (Annexure-II) under Apun
Bahan Scheme and request you kindly to circulate the same among all the H.O.Ds/D.D.O
s under your jurisdiction.

The Self-declaration form (Annexure-I) is intended for completion by eligible

employees who intend to apply for the Apun Bahan Scheme. Subsequently, these
completed self-declaration forms should be submitted to the respective D.D.Os.
Upon receiving the self-declaration forms, the D.D.Os are responsible for verifying
the provided information and subsequently completing the D.D.O Certification format
(Annexure-II). Following this verification process, the D.D.Os should ensure that both the
self-declaration and D.D.O Certification documents are promptly forwarded to the relevant
banking institutions.
