MEDICAL REPORT
RE: ABAH ENE MARY /FEMALE/ 24 YEARS
The above-named patient suffering from pneumonia, chronic peptic ulcer and polymyositis which
has led to acute pain. This disorder was diagnosed due to the patient complaints of symptoms of
immense fatigue, resounding cough, muscle weakness, joint pain, nausea, heartburn, loss of appetite
dizziness, chest and backache.
Following this diagnosis, she has been placed on rigorous regime of medical examinations.
Due to this illness, situations where extreme physical exertions, immense cold as well as environment
of rigorous activities and excitement are to be avoided. Therefore, medical monitoring by her
physician is essential for her maintenance of the illness.
In view of the above ailment, patient is hereby advised to observe medical check-up fortnightly on
appointment. We therefore crave your indulgence that patient should be relocated to Keffi, Nasarawa
State, in order for her to have access to her medical doctor for constant management and check-up.
