Rules & Regulation:
1. All application only can submit from Monday to Friday and 3 days in advance for our arrangement.
2. All sites access only allowed from Monday to Friday from 10am to 6pm (subject to the approval from property owner).
3. We only accept the application from the Telco Company by email instead of contractor or vendor.
4. Telco’s contractor is required to display the approved Telco Work Application Form (in hard copy) at site.
5. GTP Network shall not be liable for any incident during this activity if Telco’s or Contractor without any approval PTW.
6. Mandatory PPE Equipment.
a. Safety Shoes at all time.
b. Safety Helmet all the time.
c. Rigger Helmet & Full Body Hardness & Y-Lanyard (MUST BE WORN WHEN WORKING AT HEIGHT >10 FEET).
d. A minimum of two (2) person should be at the work site.
e. No work should be performed at the Tower or Rooftop during adverse weather conditions such as rain, thunderstorm,
lightning or strong wind.
f. Must have at site Fire Extinguisher, First Aid box.
g. Upon arrival at site, check surrounding using test pen/multimeter before start work and any Hazard.
7. It is supervisor or anyone equivalent responsibility to define a hazard before conduct an activity at site. Any unsafe action or
unsafe condition identified; the stop of work is applied.
8. Telco’s or Contractor are required to ensure the site are left in order & clean upon completion of work.

1. Please complete the attached PTW form and email to edgepoint TOC 3 days prior to work
commencement.
2. PTW will be processed only during office hours. Daily 0900 to 1800hrs.
3. Completed PTW to be emailed to toc@edgepointinfra.com in PDF format
4. Email subject to follow the below format to ensure it is received by relevant departments for
processing.

5. For Ground base towers(GBT), we have implemented smart lock systems. Please ensure that
access is available to the team visiting the sites.
