Scope of Works :

1. Provide manpower(20) to assist, replacement of the following

(17 days approx)
*422-AS1/AS2/AS3 Airslide Canvass

2. Provide manpower(40) to assist, replacement of the following

(17 days approx)
*Airslide Canvass at Raw Mill Area
-392-AS1 -392-AS4 -392-ASB -392-ASE -392-ASH
-392-AS2 -392-AS5 -392-ASC -392-ASF
-392-AS3 -392-ASA -392-ASD -392-ASG

*Replacement of Raw Mill Area Bag Filters

3. Replacement of Conveyor Rollers from Crusher to Raw Mill
Area

4. Declogging and Cleaning of conveyor materials from crusher

to raw mill (Including VRM Center Table Internals - Approx 3
tons and for actual assessment)

5. Replacement of conveyor side skirts at Crusher to Raw

Mill Area.

6. Clearing & disposal of materials inside the Raw

Mill, Raw Mill Cyclones, Bucket Elevators and other
materials during 2023 SPS.

7. Conduct minor repairs of equipment as needed from Crusher to

Raw Mill Area

8. Provide/Assigned personnel to properly segregate and

dispose waste materials such as cellophane, pallets,
metal, etc. to designated areas.

9. Provide 2- Foreman / leadman for the supervision of

activity, 2- Safety officer

10. Other necessary works needed to complete the Crusher, Raw

Mill and Coal Plant Area SPS Activities.

11. Provide necessary tools and equipment's to do the jobs.

Terms and Conditions:

1. Work permit shall be process & completed before start of

work. Tool box meeting should be regularly (twice a shift
 or as needed) conducted to discuss the Job planning tool.
2. All Holcim required PPE's should be provided to all
manpower by the Contractor(dust mask, gloves, safety
goggles).
3. Contractor shall provide enough manpower for the completion
of the project including cutting outfit and Oxy/Acy if
necessary.
4. The contractor should provide their own Safety inspector to
call & correct any deviation.

Note: No personnel shall render work in excess of 12 hours.

5. Consumables such as power, water, housekeeping tools will

be provided / shouldered by Holcim.
6. All personnel shall be covered with accident insurance, SSS,
and other usual benefits should be provided by contractor.
7. The contractor shall be held responsible for all damages to
person, materials and equipment that might occur during
