PARENTAL CONSENT

I/We , , the undersigned legal parents/guardians of

, hereby willingly and voluntarily give consent
to my/our daughter/son being on campus on May 24, 2024 from 7:30 AM to 10:00 AM
at Our Lady of the Sacred Heart Parish, Marigondon to participate in the
Baccalaureate Mass.

I/we have considered the necessity of this event for the students. I/we allow my
learner to participate in this event, provided that due care and precaution will be
observed based on the IATF Guidelines for MGCQ to ensure the comfort and safety of
my/our son/daughter and that STEC employees and personnel shall enforce
contingency measures in order to avoid any untoward incident that may happen beyond
their control.
