the school campus at STEC Covered Court that he/she is physically fit to participate in the volleyball tournament
as __________________________ on May 03, 2024 until 12 NN as a requirement of the subject on Health
Optimizing Physical Education.
I/we have considered the necessity of this event for the students. I/we allow my learner to participate in
this event, provided that due care and precaution will be observed based on the IATF Guidelines for MGCQ to
ensure the comfort and safety of my/our son/daughter and that STEC employees and personnel shall enforce
contingency measures in order to avoid any untoward incident that may happen beyond their control.
