PARENTAL CONSENT

I/we _______________________ hereby willingly and voluntarily give consent to the

participation of my/our son/daughter, ___________________________, to stay on the school
premises on April 15, 2024 from 7:30 AM to 2:00 PM with regards to the Team Building Activity.

I have considered the necessity of the said event. I allow my learner for this activity
provided that due care and precaution will be observed based on the IATF Guidelines to ensure
the comfort and safety of my son/daughter and that DepEd employees and personnel shall
enforce contingency measures in order to avoid any untoward incident that may happen beyond
their control.
