Dear Dr Tan,

The municipality of Panukulan has been a consistent recipient of the SGLG Project ever since
and we are humbled to be included in their project this year. However, this year’s qualification or criteria
included the requirement of Local Investment Plan for Health 2020-2022 in each municipality. The
SGLG validation team questioned our presence of LIPH because they noticed that the Provincial Office
did not have any copy of our LIPH 2020-2022 when we’ve already passed, via email, our copy to your
office last June 4, 2019. Hence, the paucity of our validation.
In line with this, we are humbly requesting from your Office a Certificate that would testify to
our compliance with the said file requested by your office. Thank you very much for your kind and
positive response.
