PART I: INFORMATION SHEET

INTRODUCTION
You are hereby invited to participate in a research study conducted by Ann K. Maniego of
Principal Investigator at Central Mindanao University, your participation is voluntary.

PURPOSE OF THE RESEARCH


The purpose of this study is to identify the effects of the PE34 Folk Dance subject on students'
emotional well-being and academic performance. By conducting this study, we aim to provide
insights into the role of folk dance in academic settings and explore its benefits for student well-
being.

TYPE OF RESEARCH INTERVENTION

If you decide to participate in this study, you will receive a questionnaire and will be asked to
answer the survey about the effects of PE34 subject folk dance on your emotional well-being and
academic performance. It may require you to provide your grade in the PE34 subject.

PARTICIPANT SELECTION
You have been selected because you are a 2nd-year student enrolled in the PE34 Folk Dance
subject at Central Mindanao University. You represent the participants whose responses will help
us better understand the effects of the subject on academic performance and emotional well-
being.

VOLUNTARY PARTICIPATION
You can freely choose to take part in the survey. There will be no penalty or loss of benefits for
either decision that their choosing to participate or not will not. If you agree to participate, you
have all the decision in answering the questionnaire that will be given.
PROCEDURES

You will be asked to answer a questionnaire about your experiences with the PE34 Folk Dance
subject. The questionnaire is divided into three categories:

1. Survey about folk dance practices in the course

2. Survey about academic performance

3. Survey about emotional well-being

DURATION

It will take you more or less 20 minutes to answer the questionnaire that the researchers that will
be given.

RISKS

This study represents low physical risk as participants will only be required to answer the printed
questionnaire. The questions in the questionnaire does not address personal topics that are likely
to cause negative emotional reactions. The interview results will not be released by the researcher
in a way which would identify you. The time commitment to answer the interview is up to the
participant.

BENEFITS

Participating in this study may help to uncover the benefits of folk dancing, both academically
and emotionally. The findings could also contribute to the broader understanding of how
traditional practices like folk dancing can positively impact the students.

CONFIDENTIALITY
The records of this study will be kept private. Participants will not be identified in any
publications that result from this study. All information collected about you will be kept strictly
confidential. Confidentiality, privacy and anonymity will be ensured in the collection, storage
and publication of research material. The data generated in the course of the research will be kept
securely in electronic form for a period of ten (10) years after the completion of a research
project.

SHARING THE RESULTS


You will be informed on the results of this study, and it will be shared on venues that would help
make the results appropriately presented. The copy of the manuscript will also be provided and
will be made available at the main campus libraries.

RIGHT TO REFUSE OR WITHDRAW

You can freely choose to take part in this survey and as much as possible your participation
should NOT be influenced by the researcher’s position and should NOT feel be threatened
should you decided NOT to join as it will not be taken against them. There will be no penalty or
loss of benefits for either decision. If you agree to participate, you may stop at any time.
PART II: CERTIFICATE OF CONSENT

By signing this consent form, I confirm that I read and understood the information and
have had the opportunity to ask questions about it and any questions I have been asked
have been answered to my satisfaction. I understand that my participation is voluntary and
that I am free to withdraw at any time, without giving a reason and without cost. I consent
voluntarily to be a participant in this study.

STATEMENT BY THE RESEARCHER OR PERSON TAKING CONSENT

I have accurately read out the information sheet to the potential participant, and to the best
of my ability made sure that the participant understands that the following will be done:
1. Answering questions of the interview as the participant of the study.
2. The Privacy and confidentiality of the proceeding activities in the conduct of the study
will be maintained and protected.
3. Keeping and destroying of the data responses, recording and transcript as specified in
the study protocol.
I confirm that the participant was given an opportunity to ask questions about the study,
and all the questions asked by the participant have been answered correctly and to the best
of my ability. I confirm that the individual has not been coerced into giving consent, and the
consent has been given freely and voluntarily.
