NON-UNION EMPLOYEES

WEEKLY PAY OPTIONS

DIRECT DEPOSIT

1. You can have all or a portion of your paycheck deposited into multiple bank, credit union
or other financial institution accounts of your choice.
2. Direct Deposit goes into effect immediately after you sign up during onboarding.

WISELY® CARD

1. You can sign up for the Wisely® Card and have all or a portion of your paycheck
deposited onto the card.
2. You can use the card to shop, pay bills or get cash.
3. There are no sign-up costs, credit checks, fees, or minimum balance. It’s your paycheck
on a card.

PAID TIME OFF

VACATION

1. Vacation is granted every six months each anniversary year, with the benefit increasing
progressively until a maximum benefit of five weeks is reached at twenty-five years of
service.
2. You will receive two weeks of vacation prior to your one-year hire-date anniversary:
a. You will receive one week of vacation after successful completion of onboarding (90
calendar days if not extended).
b. You will receive one week of vacation after six months of employment.
3. Starting on your one-year anniversary, your vacation grant will increase on a periodic
basis, reaching the following amounts:
a. You will reach three weeks per year on your five-year anniversary.
b. You will reach four weeks per year on your fifteen-year anniversary.
c. You will reach five weeks per year on your twenty-five-year anniversary.
4. You can bank unused vacation days up to a maximum of two benefit years’ worth of
grants.
5. Unused days and any current accrual will be paid off up to the bank maximum at
separation.
6. Employees can exchange one week’s worth of vacation for pay per anniversary year
provided they have at least one benefit year’s allowance left in their bank after selling
back.

2. You may be asked to work a skeleton crew on some of the holidays.

PAID TIME OFF (cont.)

SICK TIME

1. You can use sick time to be used for your own legitimate illness or medical emergency
with an immediate family member.
2. During your first year you will receive 40 hours of sick time, distributed as follows:
a. 16 hours of sick time after 13 weeks of employment;
b. 16 hours of sick time after 26 weeks of employment;
c. 8 hours of sick time after 32 weeks of employment.
3. On your first-year anniversary and thereafter, you will receive 40 hours on each
anniversary hire date anniversary.
4. Hours may be banked year to year.
5. The Company will pay off any unused sick time over 160 hours, up to a maximum of
four weeks upon your retirement.
a. You must be 59.5 years of age to qualify.
b. You must be retiring from the company.

LIFE INSURANCE

1. After 30 days of employment, you will receive coverage under group term life insurance
which is $50,000 of coverage with an additional $50,000 accidental death coverage.

TUITION REIMBURSEMENT

1. You must be working for the Company for at least one year to qualify.
2. You must obtain approval from Corporate HR prior to enrollment in the course.
3. The Company pays 50% of tuition and books for business-related courses.
4. You pay for classes at enrollment with Alro reimbursement at course completion.
5. You must achieve a grade of "B" or better for reimbursement.

DISABILITY

SHORT TERM DISABILITY (STD)

1. At first anniversary & yearly thereafter, you earn 1 week (employee's work week of either
5 8-hour days, 4 10-hour days or 3 12-hour days) of STD.
2. STD may be used only for periods of disability of at least 5 days.
3. STD is paid at rate of 75% of weekly wage.

4. STD may be “banked” year to year but will not be paid off at termination.
5. STD is a simple balance of earned minus used days.

LONG TERM DISABILITY (LTD)

1. After 90 days of employment, you are eligible for LTD through our disability insurance
carrier.
2. LTD begins if disability lasts 90 days or longer (even if STD balance could pay).
3. LTD is 60% of previous year W-2 wages to a maximum benefit of $6,000 monthly.

LEAVE

PREGNANCY AND PARENTAL LEAVE

1. After one year of employment, you may be entitled to pay directly related to the birth or
adoption of a child. You must have an approved FMLA leave related to the event.
2. If you are pregnant and have an approved leave, you will be entitled to two weeks of
pregnancy leave, paid at 75% of your payrate.
3. If you have an approved leave related to maternal or paternal bonding time or adoption
of a child, you will be entitled to two weeks of parental leave, paid at 75% of your payrate.
4. If you qualify for both pregnancy and parental leave, you will be entitled to a maximum
of four weeks of paid leave.

BEREAVEMENT LEAVE

1. You will receive pay for 3 days related to the death of an immediate family member.
2. Leave must be used within 90 calendar days of the family member’s passing.
3. The leave can be used for the purposes of grieving, attending a funeral or memorial
service, or for settling legal or financial affairs.

HEALTHCARE

CONDITIONS

1. You will be eligible for coverage related to medical, dental, vision and prescription
expense after 30 days of employment.
2. You must work 30 hours per week to be eligible for coverage.
3. The Company is self-insured & uses Blue Cross Blue Shield PPO Network.
4. If your spouse works and insurance is offered, he/she must take their employer's
insurance, regardless of cost or benefit level.
5. You must notify the Payroll & Benefits Dept within thirty (30) days of a change of status
Failure to notify Payroll & Benefits within thirty days could result in repayment of
unauthorized charges.
A change of status includes the following:
a. Marriage;
b. Divorce;
c. Birth of child;
d. Spouse obtaining employment where healthcare is offered;

e. Spouse working a different number of hours and healthcare is offered;


f. Separation (meaning couple physically no longer living together).
6. In all cases the Plan Document is controlling.
7. Enrollment is completed on-line enrollment only through MyADP.com.
8. A Flexible Spending Account (FSA, Flex or Section 125) is available to pay for non-
reimbursed medical or dependent care expenses using pretax dollars.

HEALTHCARE (cont.)

COVERAGE

Three Plans are available:

1. 90/10 Plan
a. Coverage is basically 90/10 in-network or 70/30 with deductible out-of-network.
b. Dental – 100% preventive; 80% for basic services; 50% for major restorative work.
c. Vision - $300 every 12 months for exam, lenses or contacts, or frames.
d. Coverage includes prescription (Rx) card with following co-pays:
i. Generic $10.00 co-pay for 30-day retail supply.
ii. Brand - no generic available – 50% of cost to $40.00 maximum for 30 days.
iii. Brand – 50% of cost to $50.00 maximum for 30-day supply.
iv. Specialty – 25% of cost up to $100 maximum.
v. For 90-day mail order supply, cost is one generic or brand co-pay as above
e. Your Weekly Cost: for Single: $20.50; 2-Person: $41.00; Family: $61.50.

2. 80/20 Plan
a. Coverage is basically 80/20 in-network or 60/40 with deductible out-of-network.
b. Dental – 100% preventive; 80% for basic services; 50% for major restorative work.
c. Vision - $300 every 12 months for exam, lenses or contacts, or frames.
d. Coverage includes prescription (Rx) card with following co-pays:
i. Generic $12.00 co-pay for 30-day retail supply.
ii. Brand - no generic available – 50% of cost to $45.00 maximum for 30 days.
iii. Brand – 50% of cost to $55.00 maximum for 30-day supply.
iv. Specialty – 25% of cost up to $150 maximum.
v. For 90- day mail order supply, cost is one generic or brand co-pay as above
e. Your Weekly Cost: for Single: $10.50; 2-Person: $21.00; Family: $31.50.

3. 70/30 Plan
a. Coverage is basically 70/30 in-network or 50/50 with deductible out-of-network.
b. Dental – 100% preventive; 80% for basic services; 50% for major restorative work.
c. Vision - $300 every 12 months for exam, lenses or contacts, or frames.
d. Coverage includes prescription (Rx) card with following co-pays:
i. Generic $15.00 co-pay for 30-day retail supply.
ii. Brand - no generic available – 50% of cost to $50.00 maximum for 30 days.
iii. Brand – 50% of cost to $60.00 maximum for 30-day supply.
iv. Specialty – 25% of cost up to $200 maximum.
v. For 90-day mail order supply, cost is one generic or brand co-pay as above

RETIREMENT PLAN

1. Rollovers from previous employer plans are accepted immediately.


2. You may begin contributing to 401(k) immediately upon hire, but with no company
match.
3. After one year and 1,000 paid hours, subject to annual Board of Directors Approval, you
will begin to receive Company contributions, which include:
a. Safe Harbor contribution (3% and 100% vested after 1 year).
b. 401(k) match (50% of the first 5% contributed by employee w/6-year vesting).
c. Discretionary contribution – (0% to statutory limit based on Board vote w/6-
year vesting).
4. The plan year runs same as the fiscal year, 6/1 through 5/31.
5. Vesting for 401(k) and Discretionary is “step” type – 100% vested after 6 years in plan.
6. John Hancock Retirement Plan Services is the administrator of our plan.
a. Self-directed investment through selection of mutual funds.
b. Morningstar® Retirement Manager (will walk you through how much to save, where
to invest it and options for managing your money going forward)
c. John Hancock Personalized Retirement Advice (a truly personalized savings and
