INTERZONE BASKETBALL TOURNAMENT 2024 at BARANGAY SAN VICENTE GORONG-GORONG,
NABUA, CAMARINES SUR. I understand that my child is responsible for his behavior at all times and agreed not to
hold the Barangay Officials, SK Officials, and Management for any happenings incurred because of participation in the
league.
I agree that in the event of injury, the management may act on behalf in obtaining medical treatment for my child. I
understand that my child’s signature in this consent will bind him to abide by the same rules given by the management,
and that I will abide by the same rules with my signature below.
