of Dar-es-Salaam doth hereby solemnly take oath and state as follows;

1. That, I am an adult person, sober and sound mind and as such I am

competent and conversant with what I state in the subsequent
paragraphs herein.

2. That, I am FLORAH JUMA MACHENJE as accurately spelled names

as they appear in the Voting Identity Card, registered as ID
NumberT-1003-9153-513-7.

3. That, I am FLORA JUMA as accurately spelled names as they appear

on my ID CARD NO: 203009, HYATT REGENCY DAR-ES-
SALAAM.

4. That in view of the above stated facts I hereby swear and state that,

5. I hereby declare further that, I shall at all time hereinafter and in all
records, deeds and instruments in writing and in all actions and
