INFORMED CONSENT

Dear Participants,

We are writing this letter to invite you to participate in our research study. Currently we
are grade 12 students from HUMSS strand. Participating in this study is absolutely
voluntary, and you are free to decline or withdraw your consent at any time.
The purpose of this research is to check and know if the top students in Negros Oriental
High School has the support of their parents through holistic development and to gather
the respective information that we wish to gather in this research study. You will be
interviewed on camera, which should take approximately 5 –20 minutes of your time for
this interview. All information provided will be kept with utmost confidentiality, and your
identity will be protected. We will assign codes to your responses, and only we will have
the key to indicate which number belongs to which participant. In any publications or
presentations, we will use a pseudonym, and we will not reveal any personal details
that could identify you.
By participating in this study, you will help us to better understand “The Effects of
Technology on Learning experience of senior high students at NOHS.”, and
your contribution will be invaluable. The risks of participation are Time consuming,
information risk, and social risk, but we will take appropriate steps to minimize this risk.
If you do not wish to continue, you have the right to withdraw from the study at any
time, without penalty.
