PROCEDURE FOR PHYSICAL EXAMINATION OF HEALTH RELATED COURSES

(Medicine, Nursing, Pharmacy, Med. Tech., Rehabilitation Sciences, Biochemistry, Nutrition & Food
Technology)

1. At least 5 days before the date of your Physical Examination, do the required lab test
and Chest Xray at the UST Hospital. Have the consent form signed by parent/legal
guardian.
2. On the day of your physical exam, present your ID/reg. form and lab test result at the
Information counter. Bring baby book/immunization record.
3. Proceed to nurse (room 112, cubicle) for vital signs (BP, pulse, temp.)
4. As per nurse’s instruction, proceed to doctor for physical examination and vaccine
recommendation.
5. Pay the required vaccines at the Treasury Dept., 2nd floor Main Building.
6. Go back to nurse for vaccine administration.
7. Result of your physical exam will be forwarded to the Dean’s Office. If incomplete,
follow-up at UST Health Service.

How to obtain clean catch urine / Midstream urine

1. Wash hands & genitalia before urine collection.


2. Collect *midstream urine into a sterile container.
3. Position the container a few inches away from
the genitalia & urinate into the container until half
full.
4. Finish urinating into the toilet bowl.

• Do not get on early morning urine sample.

• Make sure that you are well hydrated prior to


urine collection.
*midstream: discard the 1st few drops of urine prior to
collection of urine sample.
