DEED OF ASSIGNMENT OF SHARES OF STOCK

KNOW ALL MEN BY THESE PRESENTS:

V (the “Assignor”), a stock corporation duly organized and existing under the laws of the
Republic of the Philippines, and with principal office address at V, San Pablo City, Laguna,
herein repres, as shown by the secretary’s certificate for the purpose hereto attached marked as
Annex “A” and made an integral part hereof, do hereby assign, transfer and convey, absolutely
and unconditionally all its rights, interests and title to ONE (1) share of stock in (the
"Corporation") in favor of (the “Assignee”).

The Corporate Secretary or transfer agent of the Corporation is hereby authorized and
instructed to effect the necessary transfer of the share to and in the name of the ASSIGNEE in
the books and records of the Corporation, and any and all rights, title and interests in and to the
share, including but not limited to the right to vote and receive dividends, shall thereafter pertain
to the ASSIGNEE.
