JOINT AFFIDAVIT OF COHABITATION

We, AAA and BBB, both of legal age, Filipinos and residents of ___________,
Quezon City, after having been duly sworn to in accordance with law, hereby depose
and state:

1. That, we are common-law spouses;

2. That we declare under the law on Perjury that we have exclusively lived
together like husband and wife for over twelve (12) years now, with no legal
impediment on our part as we are both single;
3. Out of our cohabitation, we produced two children, in the order of their
birth as follows:

a) CCCC who was born on November 2, 2005 at Frisco Lying-In Clinic in

Barangay Damayan, Quezon City; and

b) DDDD who was born on June 26, 2007 at Frisco Lying-In Clinic in
Barangay Damayan, Quezon City.

Copies of their Certificates of Live Birth are hereto attached as

ANNEXES “A” and “B” hereof.

4. That being happy and compatible with each other, we desire to marry
each other and in order to ratify and legalize our union, this time as true husband and
wife under the Family Code.

That we are executing the foregoing in order to attest to the truth of the
foregoing and in order to apply for exemption from the requirement of marriage
license, pursuant to Article 34 of the Family Code.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 18 th day
