EXECUTIVE ORDER NO. 12

“AN ORDER ESTABLISING REFERAL REHABILITATION DESK OFFICIER

ORGANIZATION OF HOUSE CLUSTER AND DESIGNATION OFNCLUSTER
LEADERS OF BARANGAY BARAOIDAN, GATTARAN, CAGAYAN.”

WHEREAS, Section (3) (A) of DDB Regulation No. 2 series of 20016 entitled “OPLAN
SAGIO” Guidelines on Voluntary Surrender off Drug Users and Dependents, and Monitoring
Mechanism of Barangay Anti-Drug Campaign mandates that;

WHEREAS, the whole process of voluntary surrender to the authorities be properly

documented by the Duty Officer (DO) of the officer where the individual surrendered and.

WHEREAS, Each Local Government Unit consequently should provide the names and
offices of Duty Officers which shall be forwarded to the DDB and DILG for record purposes.

WHEREAS, Section 6(A) (3) of DDB Regulation No. 3 s. 2017 entitled :Strengthening the
Implementation of Barangay Drug Clearing Program”, provides for the organization of house
cluster with designated cluster with cluster teacher in each barangay. The cluster leader shall
directly report to the BADAC Chairman all essential factor any illegal drug activities within
his or her cluster, such as names of pushes drug related data: and

WHEREAS, Executive Order No. 037, Series of 2017 dated November 28, 2017
entitled :AN ORDER ESTABLISHING REFERAL REHABILITATION DESK DUTY
OFFICER, ORGANIZATION OF HOUSE CLUSTER AND DESIGNATION OF CLUAS
LEARDERS OF BARANGAY BARAOIDAN GATTARAN, CAGAYAN” was issued by
the DILG Memorandum Circular No. 2017-151 dated November 10 2017 MONITORING
COMPALIANCE ON CERTAIN PROVISIONS OF DANGEROUS DRUGS BPARD
REGULATION NO. 4 SERIES OF 2016 AND DOB REGULATION NO 3, SERIES OF
2017.

NOW, THERFORE, I Gerry C. Rabi Punong Barangay of Barangay Baraoidan, Gattaran,

Cagayan by virtue of the power vested in me by the constitution and the law, do hereby order.

Section 1. Designation of the Barangay Duty Officer. That AILENE BUMAGAT the
Barangay Health Worker (BHW) of Barangay Baraoidan, Gattaran. Cagayan shall be the
concurrent at Barangay Rehabilitation Referral Desk for the purposes of this order.
 SECTION 2. Duties and Function of DESIGNATED BARANGAY DUTY
OFFICER (DO).

A. Mans the rehabilitation Referral Desk

B. Conducts an interview to solicit personal information from the surrenderer.

C. Immediately inform the Punong Barangay in case the surrenderer if included in the
Target List, Wanted List and Watch List Personalities of law enforcement agencies,
such as but not limited to PDEA, PNP and NBO ir has any other pending criminal
case.

D. Refer or endorse drug dependents to the city/Municipality Anti-Drug Abuse Council

(CMADAC) for screening using the alcohol, Smoking and Substance Involvement
Screening Test (ASSIST).

E. Log and maintain a record book containing all personal information fo surrenderers.

F. Ensure confoidentialituy of all information of surrenderers.

G. Monitor progress of surrenderers, and

H. Perform other duties and functions as may be deemed necessary.

SECTION 3. Designation of Cluster Leaders. There shall be designated Cluster Leaders of

Each house cluster within the Territorial Jurisdiction of Baranagay Baraoidan

 SECTION 4. Duties and Functions of DESIGNATED CLUSTER LEADER.

5.1. Attend BADAC meetings, upon instruction of the Punong Barangay.

5.2. Maintain or prepare a list of individuals or residents in every household within the
assign cluster,

5.3. Immediately inform or report to the Punong Barangay / BADAC Chrisman of

personalities and or activities and

5.4 Perform other functions as may be assigned by the BADAC Chairman.

SECTION 5. Organization of House Clusters. There shall be Cluster or group of

houses within the barangay by any bit limited to the following.

 By the Number of Houses

 By Street
 By Purok or Sitio

Map of the houses in every cluster and maintain a list or name of household member or
dinividiuals in each luster.

SECTION 6. EFFECTIVITIY – this Memorandum Circular shall take effect

immediately.
