depose and state the following:

1. That I and my common-law spouse, LEONIVEVE L. LIMBAGA, had a

child in the person of CASSANDRA LIMBAGA LEONI who was born on
February 19, 2022, in Zamboanga City, Philippines;

2. I am seeking to utilize my surname LEONI as the registered surname of

my daughter CASSANDRA LIMBAGA LEONI in her Certificate of Live
Birth/Report of Birth, pursuant to RA 9255;

3. That I am filing this Affidavit at the Local Civil Registry/Philippine

Foreign Service Post of Zamboanga City, Philippines in accordance with
RA No. 9255 and its Revised Implementing Rules and Regulation;

4. That I execute this Affidavit to attest to the veracity of the foregoing facts
based on my personal knowledge and authentic records and for whatever
legal purpose it may serve.
