AFFIDAVIT OF CONSENT
I, IMELDA ALCARAZ, of legal age, Filipino citizen, with residence and
postal address at 249 Anabu 1-F, Imus City, Cavite, after being duly sworn to in
accordance with law, do hereby freely states, THAT:

1. I am the administrator and lessor of a property particularly located

at Reyes St,, Anabu 1-F, Imus City, Cavite which is covered by the local
government road widening program;;

2. That as due compliance to the local ordinance with regards to the

two (2) meters setback and jurisdiction with regards to the demolition of the
property including the necessary clearing and safety standards. We give our full
consent and authority to the local government to construct the pathway within
the property and that we will remove all obstructions thereto to facilitate the
construction;
3. Furthermore, the subject property has no violation other violation
with regards to property line, local boundaries and public access including roads,
waterways and has duly complied with the provisions of the National Building
Code of the Philippines (PD 1096);

4. I hereby freely execute this Affidavit of Consent to attest to the

truthfulness of the foregoing statement for the information of the local
government office and for whatever legal purpose it may serve.
