AFFIDAVIT OF DEMOLITION
I, RODERICK C. BIAZON, of legal age, Filipino, with residence and
postal address at Blk. 2, Lot 23, Jupiter St., Sunnydale Classic Homes, Alapan 1-
A, Imus City, Cavite, after being duly sworn to in accordance with law, do hereby
freely states, THAT:

1. I am the Building Contractor of the property particularly located at

Lot 11F and G, Zapote II, District I, Bacoor City, Cavite which has been applied
for a Demolition Permit;

2. That we have duly complied with the local ordinance and

jurisdiction with regards to the demolition of the property including the
necessary clearing and safety standards. Attached hereto are the pictures of the
property before and after the demolition marked as Annex “A” and Annex “B”;
3. Furthermore, the subject property has no setback violation with
regards to property line, local boundaries and public access including roads,
waterways and has duly complied with the provisions of the National Building
Code of the Philippines (PD 1096);

4. I hereby freely execute this Affidavit of Demolition to attest to the

truthfulness of the foregoing statement for the information of the local
government office and for whatever legal purpose it may serve.
