AFFIDAVIT OF SUBSISISTING MARRIAGE

I, DANIEL MKAMI NYAHUCHO, Adult, Christian and Resident of Dar es Salaam, Tanzania,
DO HEREBY SWORN and STATE as follows: -

1. That, I am a deponent herein thus conversant with the facts deposed here under.

2. That, on the date of …. I contracted marriage to one Esther Michael Matinde resident
of Dar es salaam.

3. That the said marriage was contracted under Christian rite, and witnessed by Lakus
Stanley Matari and Noela Sifael Ngowi.

4. That the marriage certicate issued in the said marriage was not registered.

5. That I intend to register the said marriage certificate to be recognized by the law.

6. That it is on the basis of the above I make this affidavit to clear any doubt or
inconveniences which might occur in any of my official dealings.

VERIFICATION

I, DANIEL MKAMI NYAHUCHO I verify that all I have stated herein above from paragraph
1, 2, 3,4, 5 and 6 are true to the best of my knowledge, and I make this declaration
conscientiously believing the same to be true accordance with the provisions of the OATHS
