SUBJECT: AUTHORITY TO ATTEND ON OFFICIAL TIME

The undersigned hereby authorizes the following personnel of the hospital to attend
Medical Committee Meeting for the Palarong Pambansa 2024 on July 3, 2024 at the
Dance Studio, Cebu City Sports Center, Cebu City:

During this period, the said personnel will be representing the hospital in an official
capacity and will discharge their duties to the best of their ability and that their conduct
will reflect the high standards of professionalism and integrity that are expected of all our
employees.

The Narrative Report shall be submitted within five (5) days after the meeting.
