AFFIDAVIT
I, JOHNNY BOY B. MORALES, of legal age, single, Filipino citizen,and a
resident of Purok 4, Brgy. Silop, Surigao City, after having been duly sworn to
in accordance with law, do hereby depose and say:
1. That I am the biological father of ABEGAIL FRANCISCO MORALES, who
was born on September 27, 2012;
2. That I am not married to the biological mother of the said child is AILEEN
BOHOL FRANCISCO who is presently residing in Manila;
3. That ABEGAIL FRANCISCO MORALES is living with me and is under my
custody and supervision since her birth;
4. That I am intending to register the Certificate of Live Birth of ABEGAIL
FRANCISCO MORALES before the Office of the Local Civil Registry in Surigao
City. However, her mother cannot affix her signature therein in view of her
present residence in Manila;
5. That in order to facilitate the registration of the Certificate of Live Birth of
my child, her mother authorized me to sign her name on her behalf;
6. That I am executing this Affidavit to attest to the truth of the foregoing
statements, and for whatever legal purpose it may serve.
