A. MOVING FROM SUPINE TO SIDE- LYING POSITION (including TURNING from BACK TO SIDE) TO SIM’S
THEN PRONE POSITION

1. Explain the procedure and its purpose.
A. SUPINE/ DORSAL POSITION 3 2 1
1. Check that the spine is in straight alignment WITHOUT PILLOW under the head and
shoulders.
2. Places the arms at the side with the HANDS PRONATED.
3. Elevates/ supports the forearms with pillows.
4. Places hand rolls (for paralyzed patients) to maintain the hands in functional
position.
5. Supports the hip joint on each side with a trochanter roll.
6. Supports the feet that the toes are pointing upward anatomical position.

B. MOVING CLOSER TO ONE SIDE OF THE BED 3 2 1
1. Slides hands and arms under the patient’s head and shoulders.
2. Pulls that section of the body towards the edge of the bed.
3. Slides hands and arms down under the patient’s hips.
4. Pull that section of the body towards the edge of the bed.
Observes good body mechanics during the procedure
7. Keeps back straight and hips and knees flexed.
8. Keeps one foot forward to give a broad base of support.

C. TURNING ON BED: BACK TO SIDE
1. Raises side rails on one side.
2. Moves to the other side of the bed.
3. Prepares pillows needed for support.
4. Positions patient for turning to the side with the head supported on a low pillow.
4.1. Crosses the patient’s far ankle over the near ankle
4.2. Flexes knee of far leg
4.3. Moves patients near arm away from the patient’s body.
4.4. Places the patient’s far arm across the chest.
5. Grasp the patient behind the far shoulder and hip.
6. Rolls the patient toward you.
7. Assist the patient until he/she is in the correct side- lying position.
7.1. Tucks a pillow on the patient’s chest.
7.2. Flexes underlying arm unto the pillow used for the head.
7.3. Puts hand rolls in place if needed.
7.4. Flexes top leg and brings it slightly forward to help provide balance.
7.5. Places a pillow lengthwise under the top leg to keep legs separated and to
support the top leg.
7.6. Supports feet at right angles to prevent plantar flexion and foot drop.

D. SIDE- LYING/ LATERAL POSITION 3 2 1
1. Positions patient on the side with the head supported by a low pillow.
2. Tucks a pillow along the patient’s back.
3. Brings the underlying arm forward and flexes it unto the pillow used for the head.
4. Brings the underlying arm forward and flexes it unto the pillow in front of the body.
5. Puts hand rolls in place if needed.
6. Flexes the top leg and brings it slightly forward.
7. Places a pillow lengthwise under the top leg.
8. Supports the feet at right angles to prevent plantar flexion and foot drop.

E. SIM’S POSITION 3 2 1
1. Position patient on the side with the head supported by a pillow.
2. Supports the upper half of the body with a pillow and places upper arm and legs
over it.
3. Places support as needed for the trunk:
3.1. A folded towel under each shoulder prevents slumping of the shoulders.
3.2. A flat pillow under the abdomen of a female with a large breast.
4. Positions arms in a comfortable position by placing on the sides or flexed at elbow
with hands near the head.
5. Places hand rolls if needed.
6. Position the feet in dorsiflexion.
6.1. With tall patients, the feet are extended beyond end of mattress.
6.2. With shorter patients, a roll is placed under the ankles.
