AFFIDAVIT

I, PATRICIO NORBERTE TABABA, of legal age, Filipino, single, and a resident of P-

13B, Barangay Pianing, Butuan City, after having been duly sworn to in accordance with law,
depose and state:

1. That I am the brother of the late ARTURO NORBERTE TABABA who died on February 20,
2016 ;
2. That he was married to the late LETECIA OMBI-ON on July 24, 1979 at Cauayan, Negros
Occidental and did not have any legitimate children;
3. That he was also married to the late ESTRELLA DAIPAL on May 26,1981 at Toboso, Negros
Occidental and bore one child named, MELVIN DAIPAL TABABA;
4. That he was then married to SUSITA H. VILLASOR on January 23, 1997 at Cadiz City and was
gifted with five (5) children, namely ANDREBON VILLASOR TABABA, CHRISTIAN
VILLASOR TABABA, JOHN MICHAEL VILLASOR TABABA, JUDY-ANN VILLASOR
TABABA and CARMEN VILLASOR TABABA;
5. I am executing this affidavit to attest to the veracity of the foregoing for whatever legal purpose it
may serve.
