AFFIDAVIT OF INFORMANT

I, RICARDO L. BALDONADO, of legal age, Filipino, married, and a resident of P-13A

San Vicente, Butuan City, under oath, hereby depose and state that:

1. I am the uncle of ROMULO L. BALDONADO, who was born on July 17 1984 at Baan
Riverside, Butuan City to the mother ALIJANDRINA L. BALDONADO;
2. I am the one processing the application for Late Registration of Birth of the aforementioned child;
3. The child’s parents are not married, and the name and whereabouts of the child’s father are
unknown due to absence of any means of communication;
4. The child’s birth was attended by CONCEPTION GATO a Hilot at San Jose Street, Butuan
City;
5. The child’s birth was not registered within 30 days from the time of the child’s birth due to
negligence and financial constraints;
6. The child was baptized and immunized;
7. I am executing this affidavit to attest to the veracity of the foregoing and to support the
application for Late Registration of Birth of the child, ROMULO L. BALDONADO.
