Reason for Referral: Mr. Fandialan was referred to explore his Personal History (Past
or Previous History)

BACKGROUND INFORMATION: Mr. Fandialan is currently studying at Polytechnic


University of the Philippines taking up BS Psychology. He is currently in his 3 rd year.
His current address is at 2737 Barangay 313, Sta. Cruz, Manila. However, he is
originally from Alaminos, Laguna. In case of any emergencies, please contact Maria
Leonora H. Fandialan at 09277333198.

A. PRE-NATAL DATA (The life of the individual starts at the zygote formation):

Mr. Fandialan, the patient, was conceived and born when his mother was 31 years old.
He is the family's second child, with one older brother and one younger sister. The
eldest sibling's pregnancy interval was around a year and a few months, while the
youngest's was about 7 years. The mother did not have any non-infectious diseases or
get any infectious infections while she was pregnant. Her blood type is O, and she has
Allergic Rhinitis. The mother received normal prenatal care, including OB-GYN check-
ups, and took prenatal vitamins on a regular basis. During her pregnancy, she did not
use any illegal substances or drink alcohol. There were no dangerous physical agents,
such as X-rays or harsh electric shocks, used. Furthermore, the mother had no major
mental or emotional distress throughout Mr. Fandialan's conception.

B. FIRST YEAR OF LIFE AND INFANT FEEDING PROBLEMS:

Mr. Fandialan was delivered naturally, with no difficulties or surgical intervention. His
delivery took place in a hospital, and he was breastfed without any problems. His
mother was a working mom at the time, but she made sure he was adequately cared
for by hiring a babysitter and having his father present. Mr. Fandialan was carried and
fondled frequently, especially at feeding times. There were no cases of refusal to suck
while nursing, and he was breastfed without incident for six months. He was never
abandoned because someone was always there to look after him while his mother
worked. The person providing care exhibited a compassionate approach. Several early
physiological development milestones were noted, including focusing his eyes,
responding to sound, playing with objects, and language development. Mr. Fandialan
is said to have learnt quickly, probably as a result of watching learning and development
videos.

C. TWO TO THREE YEARS OF LIFE (Toilet Training)

Mr. Fandialan did not intentionally or rebelliously move his stool or urinate anywhere
during the second and third years of his existence. Despite parental displeasure, there
was no pleasure-seeking conduct such as playing with feces or smearing urine. Mr.
Fandialan's father, mother, and other caregivers were strict in teaching him proper toilet
usage, focusing on flushing and handwashing. He was neither stubborn nor resistant
to toilet training, and there were no harsh punishments. There were no cases of
diarrhea or constipation during these years, and no medications were prescribed. Mr.
Fandialan did not have regular temper tantrums or outbursts. His parents leave him
crying by himself. His description was obedient but not especially stubborn, resentful,
or too submissive. At this age, destructive behavior was not noticed. Correction and
discipline were implemented tactfully. Language development was reported to be rapid
throughout this stage.

D. THREE TO FIVE YEARS UP TO PUBERTY: (Accidental experiences and early


social relations)

Mr. Fandialan shown no shame, disgust, or embarrassment about his genitals while
showering, and genital cleanliness was not stressed. There were no remarks or jokes
about genital parts made while dressing or undressing him. Because of the arrival of
siblings, he was neither taunted or compared to other youngsters. There were no
obvious symptoms of resentment toward family members. He was not overindulged,
and he was not subjected to excessive kissing or teasing of his genital parts. There
were no reports of him as a child playing around with his sexual parts. He reportedly
showed proud interest on his genital and was not ashamed of it. There were no
instances of him spotted masturbating, and no consequences or handling were
reported. At one time, he expressed interest in sex differences. When he had questions
about sex, pregnancy, and delivery, his parents were honest about it. He was neither
enticed into sexual play by other youngsters, nor did he see parental sexual
intercourse. There were no reports of excessive affection or overindulgences by
parents or siblings. Severe punishments were not used frequently, but reprimands and
explanations were given for inappropriate behavior. The loss of family members would
be painful for him. He had no loss of appetite as he was a voracious eater. Interacted
normally with the other kids. At this point, language development has been described
as fantastic.

EDUCATIONAL HISTORY

Mr. Fandialan started school when he was four years old, at Ramirez Nursery School.
He fit in nicely with his peers and playmates, at first being friendly but also shy. He
received high scores and was promoted on a consistent basis. He had a keen interest
in basketball. Escaping from school, lying, cheating, stealing, violence to other
youngsters, or sex delinquencies were not reported. Mr. Fandialan was a hard worker
who was ambitious about education. His upbringing and desire to study human
behavior may have affected his environment. The pursuit of the best education led to
school transfers as he advanced from preschool through elementary, high school, and
college.

HOME ENVIRONMENT

Life at home was not always happy for Mr. Fandialan, as they have faced difficulties at
times, which is typical for a family. Mr. Fandialan was allowed to have friends come at
home and he was given tasks around the house, such as sharing household chores.
He is currently living alone. Mr. Fandialan had a happy childhood despite adversity. The
family did not move or relocate homes regularly. Mr. Fandialan gets along with and is
liked by both of his parents. Other unidentified family issues are referred to as personal.
