SUBJECT : SAFETY INSPECTION REPORT- 02

Reference to the above subject project and our safety inspection at the site, we have noticed the
following safety issues;

1. There are inadequate welfare facilities for workers, particularly in the rest and eating areas. The
shelter provided for rest and eating is not properly enclosed, Air-conditioning facilities are not
provided, despite the arrival of summer. (See Photo 1)

Ensure that the rest and eating areas are properly enclosed, this can include installing proper
walls, windows, and doors. Provide adequate air conditioning facilities, particularly during the
summer months.

2. (Repeated Observations) It was observed that 'plug top' extension sockets were being used for
the power connection instead of industrial-type plug and socket connections at the workplace.
(See Photo 2)

Replace all the plug top type extension board with industrial-type plug and socket connections;
Schedule regular electrical inspections to verify the safety and compliance of all power
connections

3. It was observed that a water tank was improperly installed at the workplace. Empty metal
containers and loose hollow blocks were used as support stands for the tank. This makeshift
arrangement poses a high risk of collapse, compromising the safety of the workers and the
stability of the water supply system. (See Photo 3)

Ensure that the water tank is installed on a stable and secure foundation. Use appropriate
materials and construction methods to create a robust support structure. This may include
concrete bases or properly anchored steel supports.

4. It was observed that workers were using an improper makeshift working stand for column
reinforcement steel fixing. Specifically, loose hollow blocks were being used as a support stand.

(See Photo 4)

Stop the use of makeshift stands and all work in this manner should be halted until proper
safety measures are in place. Remove all loose hollow blocks being used as support stands
from the worksite.

5. it was observed that a makeshift table saw was being used without a blade guard and an
emergency stop button. a handheld circular saw was improperly installed under the makeshift
table and being used as a table saw. (See Photo 5)

Ensure that a properly designed table saw with an appropriate blade guard and an emergency
stop button is used

6. it was observed that there was no water filter system provided with the drinking water cooler
station at the workplace. Providing safe, clean drinking water is essential to ensure the well-
being and productivity of employees. (See Photo 6)

Take immediate corrective action to address the absence of a water filter system at the drinking
water cooler station, this includes installing a certified water filter system to ensure the provision
of safe and clean drinking water.

7. it was noted that there was no valid certified first aider available at the workplace. The absence
of a certified first aider poses significant risks to the safety and well-being of employees in the
event of accidents, injuries, or medical emergencies.

The contractor is required to ensure that at least one designated employee at the workplace
possesses a valid and up-to-date certification in first aid from a recognized training provider.

8. it was observed that flexible power cables were joined inappropriately, with splices and tape
used to extend the power cable. Improperly joined cables can over heat, expose live wires,
increasing the risk of electrical shock to employees. (See Photo 7)

Use appropriate connectors and junction boxes for joining power cables to ensure secure and
stable connections, and ensure all electrical works performed by qualified electricians following
established safety standards and codes.

9. it was observed that improper concrete washout practices were being followed at the
workplace. Concrete waste and slurry were disposed of inappropriately, and no designated
concrete waste collection area was provided. Concrete waste and slurry can contaminate soil
and water sources, leading to adverse environmental impacts. (See Photo 8)

Establish a designated concrete washout area at the workplace, regularly remove and properly
dispose of collected concrete waste and slurry in compliance with environmental regulations.

10. it was observed that there was an improper working platform for work at height related to
column work. The working platform was missing guard rails, access ladders, and adequate
support bracing. (See Photo 9)

Ensure that all working platforms on scaffolds are equipped with guard rails on all open sides to
prevent falls, ensure that all scaffolding structures are equipped with adequate support bracing
to maintain structural stability, conduct regular inspections of scaffolding structures by qualified
personnel to identify and rectify any safety issues.

The contractor is advised to take necessary action immediately.
