AFFIDAVIT OF ASSUMPTION OF RESPONSIBILITY

do hereby depose and say THAT:

virtue and under the laws of Japan and has engaged the services of (name of new
partner agency), a duly licensed landbased recruitment agency by the Philippine
Overseas Employment Administration (POEA) as our legal representative;

2. I likewise UNDERTAKE and ASSUME full and complete responsibility for all
consequences which may arise in connection with the said registration as well as
those arising from the previous deployment of the principal from former accredited
PRA;

3. I am executing this Affidavit in compliance with the requirements for the application
of POEA Registration pursuant to rules and regulations set by the POEA.
