GUIDELINES ON PREPARATION OF BRIDGING DOCUMENTS FOR

❖ Background
➢ At the time of lining of contract’s there could be gap between in the HSE Management System of Contractor vis
a vis IOCL HSE Management System.

➢ All Such gaps to be identified and bridging document must be prepared.


➢ A bridging document between IOCL and contractor covering Health, Safety and Environment issues is crucial to
ensure contractors align with the Safety and environment standards set by the IOCL corporation and any
differences may be resolved and procedure agreed before work commences.

➢ SOP (of IOCL & Contractor) which prevail should be controlled through the bridging document.

➢ OISD –GDN-206 (Guidelines on Safety Management System in Oil and Gas Industry) was last revised by OISD
in Aug-2023. In last revision it has been mentioned that in case of hired Services, SOP (of the organization and
Contactor) which will prevail should be controlled by through bridging /Interface document.

❖ Introduction
➢ Provide an Overview of the organization’s Commitment to HSE
➢ The Purpose, Specific Objective and Scope of the bridging document must be clearly stated.

➢ Our Health, Safety and Environment policy is approved by the Board of Directors and be updated periodically as
and when required.
➢ The policy statement clearly specify the intentions and commitments on part of top management.
➢ The policy statement made available to all stakeholders including employees.
➢ It has been displayed at all locations prominently.
➢ Contractors and business associates should be encouraged to follow Health, Safety and Environment policy.
➢ Contractors should also provide the Safety performance records to the Corporation.

➢ Commitment of top management towards Health, Safety and Environment shall be transmitted to all organizational

➢ Prompt corrective actions are to be taken for deficiencies.
➢ Procedural violation shall not be tolerated.
➢ Statutory rules and regulations shall be complied with as applicable.
➢ It should be ensured that incidents are investigated and accepted recommendations of the investigation committees are
implemented.
➢ Management audits the system for assessing performance and improving effectiveness.
➢ It should be the endeavour of the management to involve all employees, contractors, security personnel and business
associates related to our operation in the safety culture.

❖ Documentation Control Procedure

➢ Specify the document control process including amendment & reissue and document holder responsibilities.

➢ Must include contact number and date, details of Contractors and Installation etc.

➢ Clearly State the documentation requirement for HSE compliance.

➢ Establish the System for maintaining and sharing relevant issue.

➢ Confidentiality/ Non disclosure agreement shall be maintained.

A. Access Control:

1. Deployment of security posts at project construction site for access control.

2. No workers shall be allowed for entry to work sites inside Refinery without safety Induction
Training as per extant practice and records to be maintained.

3. No workers shall be allowed to enter to any operating units inside Refinery.

4. Responsible senior personnel of M/s Technip to be identified and shall be made available on
round the clock basis for the job supervision for works beyond normal working hours. Round
the clock safety coverage shall be ensured by deployment of adequate nos. of site safety
Supervisors and HSE managers compliant to Odisha Factory Rules-1950. Monthly roster of the
deployed Officers of M/s Technip along with contact details shall be shared to F&S at the
beginning of the month.

5. Arrangement for adequate illumination at the construction site shall be ensured.

B. Compliance to Work Permit System:

1. Project works shall be segregated with physical barrier of standard height with adequate
provisions of Hydrocarbon detectors along with audio visual alarms shall be provided in the
barricaded area. These detectors shall be calibrated once in 6 months and periodic testing
of beacon and hooters to be done to check the functionality.

2. Work Permit shall be issued in YPMS (in SAP) complying the provisions as per OISD-105.

3. Scanning of the area to be done for identification of all cables, marking on drawing to be
done and report to be submitted.

4. All Permit shall be issued in YPMS only. Based on the permit issued by IOCL, PMC to
implement a separate construction safety work permit format for different jobs with the
executing agency with approval from IOCL. All Permits between PMC and Technip shall be
registered at PMC-HSE. Responsibility for safety of all works carried out at PTA site to be
taken care by PMC/LSTK/IOCL-PJ and all necessary arrangement to be made in this regards.

6. A local hand operated siren shall be made available at site to alert people and stop any gas cutting activities
on receiving alarm from the HC detector. Readings of the HC detector shall be logged in register every 4 hours.

7. PMC and contract execution agencies shall maintain the work permit records of their areas and shall ensure
strict compliance of permit condition till completion of the works and permit closure.

8. All the requirements of OISD-GDN-192 / 207 on "Safety Practices during Construction” & “Contractor Safety”
shall be strictly adhered to. Compliance audit in this regard to be conducted by PMC and report to be
submitted on quarterly basis.

9. Assembly Point shall be designated and prominently displayed. Wind Socks shall be provided in the area for
guiding the personnel toward assembly point. Vehicle plying inside the premises shall strictly adhere to the
stipulated speed limits.

10. Mobile phones are strictly prohibited inside the work area. Intrinsically safe / flame proof mobile phones
authorized by IOCL shall only be used.

11. Ensure 100% use of job specific PPEs, Coverall by all Contractor workers with Safety Helmet (as per IOCL’s
Colour Code) and IFR suits for hot job gang.

12.All electrical equipment and facilities, temporary electrical connections used in the construction sites shall be
tested and certified by a competent person. Scaffold certifications before use is a mandatory compliance
requirement.

13.Vegetation growth to be periodically cleaned near the project declared area.

14. Job involving Road blocks, road blocks during marching of crane and radiography jobs shall be informed to
nearby units, PDR RSM & F&S by PMC.

15. Mock exercises on actions in case of HC leak/rescue of workers etc. shall be conducted once in a month.

C. Welfare:

1. Adequate rest shelters, cycle parking area, Rest Shelter, drinking water facilities and toilets
separately for lady and gent workers shall be made available.

2. Health check-up records of all contract workers engaged in the project area to be compiled in line
with HR requirement.

3. Round the clock availability of emergency vehicles shall be ensured for the to and fro movement
up to IOCL first Aid centre.

Penalties for non-observation of safety norms:


The following penalties shall also be considered with a view to improve the safety aspects of execution of the job
based on the job requirements:
Penalties for Violation / Non-adherence of safety procedures and practices:
1. Violation of applicable Safety, Health and Environment related norm- a penalty of Rs.5000/per Occasion
2. Violation as above resulting in Any physical injury, a penalty of 0.5% of the contract value (maximum of
Rs.2,00,000) per injury in addition to Rs.5000/per occasion as in item 1.
3. Fatal accident, a penalty of 1% of the contract value (maximum of Rs.10,00,000) per injury in addition to
Rs.5000/per occasion as in item 1.
4. The contractor is advised to take appropriate insurance policy for the effective implementation of the above
penalty provision.

In case of accidents depending on the seriousness of injury etc. in addition to the hospitalization / Treatment charges
and Group insurance amount, compensation shall be paid by the Contractor to the affected person / his family
members in presence of Engineer-in-charge as per Workmen Compensation Act.

Compliance of Personal Protective Equipment (PPE) matrix:

In addition to provisions / clauses given in General Conditions of Contract (GCC), it is obligation of successful
tenderer to abide by and ensure compliance of requirements of Oil Industry Safety Directorate (OISD) vide
OISD-STD-155(Personal protective Equipment).

Use of Inherently Fire Retardant (IFR) Coveralls by all persons entering refinery battery area irrespective of
any job (cold, hot or any job otherwise) is mandatory. The requirements are uploaded along OISD-
STANDARD-155.

Additionally, guidelines of providing PPE to their workmen during discharge of the contract is also attached
with tender document. The PPE matrix is uploaded as a part of tender document for compliance.

Specifications of the commonly used Personal Protective Equipment are attached with tender document.

In addition to provisions/norms made in General Conditions of Contract (GCC), Contractor shall also abide

❑ Quarterly Disaster Drills is a mandatory requirement for each of the IOCL’s Refineries as per Divisional
guidelines. Three (3) Onsite Disaster drills (Level-II) are conducted (One drill in odd hours) and one Offsite
Disaster Drill (Level-III) is conducted involving Mutual Aid Organizations, District Administration, NDRF and
statutory Bodies of the state. The Disaster drills are conducted as per Emergency Response Disaster
Management Plan duly approved by IOCLs Board & Petroleum and Natural Gas Regulatory Board (PNGRB) to
check the effectiveness.

❖ The existing MOU on Mutual Aid Scheme between the Industry Members (IOCL-Paradip Refinery, IOCL-Pipelines, Paradip,

three years.

❖ Oil Spill response Mock drill conducted at santra creek with the scenario of leakage of hydrocarbon
from Low Point Drain of FO Line No PB2- L2-7 at elevation of 9.410 in Tier 2 of pipe rack at Bridge”. ,
Demonstration on use of Oil dispersant chemical and Oil Containment Booms using inflatable boat
was done during the exercise

❖ Personnel Selection, Training, and Competency:

➢ HSE training requirements for contractor personnel must be specified.


➢ Define the competency standards/minimum qualification criteria for specific jobs that must be met and the
selection process.

❖ Monitoring and Auditing:


➢ HSE performance monitoring criteria must be outlined.

➢ Frequency and nature of audits to assess compliance must be specified.

Specialized safety trainings by F&S
❖ Specialized safety trainings will be imparted to all the contract workers and drivers working inside Paradip Refinery
premises. Each specialized safety training topic will be covered in a half-day training session as per the schedule given
below. The training started from 3rd June 2024 onwards.

dissemination of the safety information( Refer specimen template for Declaration in Annexure A).

➢ Any relevant forms, checklists, or additional documentation must be attached as Annexure.
